Please use our benefit inquiry form on our website, and wait for a response to your benefits PRIOR to having an appointment. If you need a prior authorization or referral, we will do our best to obtain this for you. Failure to verify benefits before an appointment can result in denial of coverage, and therefore an out of pocket expense to you.
